A new model of patient care

Thank you for choosing Dunlap Family Physicians (DFP) as your health care provider. We are excited to share with you a new concept for delivering care known as Patient Centered Medical Home (PCMH). Our new model of care is designed to provide better coordination of your overall health, inside and outside our office. We understand you may have some questions about PCMH:

 

 

 

 

Where is this new “medical home”?

Dunlap Family Physicians is the “home” of your medical care. It’s not a location or place, but rather a concept that as your primary care provider, we will be the “hub” for all your health care needs.  Navigating the world of specialty physicians, lab tests, follow-up appointments and routine checks can be overwhelming; so our team will do this for you! We are the “home” or “hub” for your care in our three office settings in Orrville, Dalton and Apple Creek.

Okay, so what is PCMH exactly?

We’re glad you asked! Have you ever felt rushed by your physician or health care provider? Have you ever felt you didn’t receive enough time to talk about other issues that may be concerning you, rather than just for the sore throat you went in with originally? PCMH is a new approach in managing the care of all our patients – we will provide you with a team dedicated to your care. The team is headed up by your health care provider, who will get to know you well and works with a team of nurses, care coordinators, pharmacists, and specialists to make sure everyone is on the same page regarding your care. This concept will provide you with ample time and resources with your health care provider and your PCMH team for complete care! (top)

How is this different than my care before?

Prior to the PCMH model of care, patients were often lost in the maze of scheduling follow-up appointments with specialists, insurance coverage questions, medication cost questions, emergency situations or receiving test results. Now, PCMH provides each patient their own team – which is led by your health care provider and consists of many important members such as a nurse, care coordinator, billing specialist, pharmacists and more. Don’t worry, we’ll explain all these roles! PCMH also promotes constant communication across all spectrums of the health care universe. Our team at DFP will be receiving updates from pharmacies, specialists, and insurers about your care.

For example, let’s say you need to see a specialist after meeting with your primary provider. Your team will help you find a specialist (such as a cardiologist, surgeon, OB/GYN or urologist) and make sure your whole team is in the loop regarding your treatment. Your team will assist in making the appointment, ensuring the specialist is covered by your insurance, and update the specialist physician regarding the need for your appointment. If the specialist orders a lab test, our team will assure the results are received in a timely manner and communicated to you accordingly. DFP will always have a provider on-call after hours. Patients are encouraged to call our after-hours line for medical questions to avoid unnecessary visits to the emergency room, however, patients are advised to call 911 or visit the nearest emergency room for medical emergencies. Do I have to pay extra for PCMH?

No – there is no cost to the patient for the PCMH model. All PCMH visits are billed as typical visits through your insurance provider. (top)

What other benefits are in it for me?

First, PCMH is a partnership between you and your health care team. So speak up! We want you to ask questions, be honest with your symptoms and trust us with your care. We encourage our patients to play an active role in their care team – tell us what you’ve been doing lately, any changes in your family’s medical history, or new lifestyle changes. Together, we will manage your care and develop goals for your overall health.

Let’s start with your routine care benefits:

We will manage your yearly check-up schedule, which includes any shots or annual tests needed. Did you forget to get your mammogram last year? Or was it difficult to schedule your colonoscopy in past years? This year, our team will assist you in your year-round, age appropriate screenings and “tune-ups”! We will take the time to learn about you and your family’s medical history, so we will make sure you’re receiving personalized care tailored to your specific needs. We will ask all patients to sign a Patient Contract; a written agreement between the patient/family and DFP specifying the role of the PCMH as your model of care.

Now, if you ever need to see a specialist:

Our offices serve as the entryway for additional medical treatment, if needed. If your knee has been hurting for several weeks, rather than searching for an orthopedic physician on your own and trying to navigate their new patient process, talk to us about your knee pain. If you and your provider feel a specialist visit is needed, your team will help you select a specialist physician and make the appointment for you. This will save you time and trouble trying to get through to a new physician.

Patients may think their family doctor is just for “colds and flu”, but we are much more than routine care. We encourage all patients to come to us for any and all issues you may have. Together, we will manage your care inside and outside our office.

And if you manage a chronic issue, such as diabetes, asthma or high blood pressure:

Are you overwhelmed by the management of your chronic issue? Your PCMH will now partner with you to ensure you have all the tools you need to manage properly. We will also take time to explain everything and align proper support and resources if needed.

And most importantly, patient centered care isn’t just about getting help when you or your family member gets sick. It is about preventing problems so you stay healthy. Patients may also receive a survey after their visit - we encourage you to complete and send back so we can evaluate how we're doing. (top)
